A National Patient Safety Agency (NPSA) Rapid Response Report focussed on this subject in January 2008. It was found that 3 deaths and over 400 patient safety incidents involving oral chemotherapy had occurred between November 2003 and June 2007. A number of recommendations were made which can be read here. The British Oncology Pharmacy Association (BOPA) have collated relevant information about oral chemotherapy on their website.
